CANCELLATION/REFUND/EXCHANGE POLICY AGREEMENT: 

Cancellation: 

Any canceled, rescheduled, or missed appointment without 48hr notice will be assessed to the following fee: $50 – Any Appointments
Refund: 
Once treatments have been PURCHASED, they are transferable to store credit ONLY unless notified otherwise. This credit can be used to purchase any other treatments and products. Refunds are only valid for medical reasons such as severe illnesses with physician’s report. In a package, if no treatments have been done, full refund is granted.
In Store/Online Purchase:
· Remaining credit is calculated from deducting used services and products.
· Debit/Credit Card/Care Credit: all refunds are subject to a 15% processing fee from the purchased package.
· Cash: full refund is granted.
Groupon/Livingsocial: 
· Skin Perfect Brothers is not responsible for refund purchases from affiliated websites such as Groupon, Livingsocial, and/or any other third-party programs. You must contact the affiliating website for refund.
· If you would like to transfer the purchased package to store credits directly, 50% of the purchased value can be refunded as store credit per manager’s approval.
